Let's all keep in mind that there are (at least) 2 different grievances here: #1. how could you screw that up? Everyone has a bad day. The Joint Chiefs of Staff would be happy to have only screwed up once. There's an old saying in private aviation, "there are only 2 kinds of pilots, those who have landed with the wheels still up, and those who haven't done it yet". #2. when the damage is known, what did you do about it? This part is inexcusable.
