Many years ago, I got set of axles from a very reputable manufacturer that were beautiful in every way, except the stud holes weren't threaded for the 1/2" studs I intended to use. Yes they were ordered for screw in studs and the studs were even in the box separately.

Oh well, easy fix I thought. After wrecking a 1/2-20 tap and not putting a scratch in the holes, I sent them back. The company apologized, fixed them and life went on.
