I've heard you have to have your original, 1st-ever SS card, and a birth certificate, but haven't looked into it.

You don't have to have your first ever SS card but you will need to have one along with your birth certificate. That was the PITA for me since I don't reside in the state where I was born, parents long since passed away and didn't have a certified copy of it.
