My understanding of the real cause of the O ring failures was NASA pushing to launch in weather condition that exceeded the lower operating temperature range of the O rings to seal properly :scope
THEY GOOFED up and killed some good people, more Government waste of people and resources
Nobody got punished for that decision that I know of, I think they should have had a full open investigation and taken appropriate actions to ensure that it never happened again
