If you have a burning desire for an NV4500 do yourself a favor and either install the fifth gear fix right up front or buy one with a good fifth gear fix already installed. If you don't, you will be.

The 5th gear nut loosening was only an issue with the Cummins trucks, not the gas versions. It has something to do with the harmonics of the engine at a certain RPM/MPH.
