The pre 2003s back to 98,5 were real good trucks with this exception.Also the 5 speeds have a bad rep for taking themselves apart if you lug them.
My die hard Guru did all kinds of terrible and wonderful things to his 2002 for many years and recently got a 2006 3500 6 speed, LOVES IT! Says hes getting 25 MPG w/90 horse injectors.Also, conventional wisdom locally says an auto just will not live behind these motors, I advise to not buy an auto at all.

I have a 2005 6 speed and love the truck I would like to know how someone is getting 25 mpg through if I really try hard I can manage 18-19 empty but you have to work real hard to do it I think 18 is good for this big of truck but that's a long way from 25.
