Looking at the b/c sheet info posted by Dan;
- hemi/4spd/4.10 had 215 fan
- 440/4spd/3.54 had 215 fan
So I think it is very likely that a 440/4spd/4.10 would also have a 215 fan, which would agree with the parts book.

Here is the '69 service manual info. It shows 4 different fans used with the 440 engine, 18x2, 18x2-1/8, 18x2-1/4 and 18-1/2x2-1/2. There are no part numbers, however with 4 fan sizes, I think it is very likely that the parts book is correct that a '69 440/4spd would have the 215 fan.
