Getting the centers made that way would be easy...just have to specify to the guys when they are cut. The center bore hole is one of the first things machined, and the rough size hole before machining should be small enough to get the desired size. (I don't remember off hand what they start at, but would vary with different manufactures as well)

As for offset, most the custom wheel companies built them to suit as it is, so just tell them the offset you want ?

Aside from CNC cut billet centers, there is also a local wheel company who does cast stuff, makes the mold, pours and machines. He'll do custom stuff and make molds to suit, but just a matter of paying for it. (we thought about it before) but the guy is just real hard to deal with I have found in person..
