In general, adapters usually change bolt pattern diameter but don't have to. An adapter always has 2 sets of studs - one to bolt adapter to axle and one for the wheel. Adapters are generally considered much weaker than a properly made spacer with correct studs. I would never run an adapter on anything other than a very mild stock vehicle.
