Open wheel - Indy & F1 - for years used a seat insert, maybe still do, and that seemed to work. In any case, the MSO bunch doesn't seem to use as much safety equipment as some of the OKC guys do.
Yes they do. They are custom fit to the driver and use space age materials IIRC.
Not the holiday inn pillows and egg crate foam MSO is using.
