The diaper has a big depends pad in it to absorb any fluid that spills. The hole for the oil level is a plastic window that is sealed. The drag link holes are open and there is a chance that fluid may escape from that location if the block or pan is windowed in that area, but a majority of the block/pan is covered and the hole is high enough to contain the contents of the pan/block. Some people run a bellows on the drag link that seals to the diaper, but I have not explored that option.
