I come up with 4028412 as

chysler 76 8-400
chysler 77 8-400
dodge 76-77 8-400
plymouth 76-77 8-400

BB-727 from a 76-77 400 engine

5191 is the build date October 17, 1975 so it's a '76 model trans.
