My car had two. One in very good, unstained condition must have been above the glove box. It was found when the glove box liner was removed. It was neatly folded twice, just like a business letter is folded.
Another in poor but readable condition was on the metal floor, under the rear carpet underlay, in front of the rear bench seat, right side. It was folded diagonally and stained by the tar-like sealer.
The one in good condition had handwriting, "Hurst", "green" (interior color), "FM" (radio), "A/C", and "WoodGR" (probably for wood grain door and quarter panel trim).
If you don't see two dolphins, you need a vacation.

Well I have looked every where except inside the rear seat backs and under the carpet behind the rear seats. Ive heard its not real common to find one in these cars but I have not finished looking..The IBM records card appears to be the way to find out more details on the options.. Some of those locations don't work on a '66 Charger or are hard to access, such as the rear seat backs. Same problem in Barracuda fastbacks.
