Is it for real? Let's say If the tranny goes away in my 9 year old Grand Cherokee will they actually pay to have it fixed or will I end up still paying a huge deductible or some kind of prorate scam?
In theory, there is no difference between theory and practice. In practice, there is.

They must promise the moon. I got a call about a month ago from a family member that wanted to get one from them. Asked me because I’m the car person in the family. My understanding of any warranty other than an oem is that they will collect the money for policies, then go bankrupt, claim you didn’t do proper maintenance, make you use only their “network” that you can get 8 to for a month, etc. or use crap parts. I’ve heard nothing good about these things from any company.
I recall well a friend getting a set of tires when he worked on the road. Took a Gemini warranty at point of sale. He was home and had a flat or something and the local place, with a permanent Gemini sign hanging prominently on the top of the building, telling him to get lost as it wasn’t covered.
I want my fair share

Most aftermarket warranty are garbage. You're better off just putting the money in a Mason jar.

my local dealer for GMC's ( the service writer) says, some aftermarket warranties are pretty good

When you hear testimony "They paid for my replacement transmission" its either a fake review or its one of the very VERY VERY few that actually got good service. The exception and not the norm. Sometimes the parts to replace are covered but not the labor. Think head gaskets. They pay the $175 for gaskets but not the $2900 in labor. All of them should be treated as garbage.

Common sense tells me that if they are making money on selling something then the consumer will lose most of the time. Well, that’s how insurance works. You spread the risk out over all your clients while they pay to be covered. Health, fire, auto liability. Most of us can afford to spend $1000/yr for auto liability, few of us can afford to pay someone’s medical bills we injure.

20-some years ago I bought a warranty for a used car I'd bought because it's exhaust manifolds were known to crack. Sure enough, they did, and the warranty paid for replacements, minus the deductible. Bottom line, I was ahead on the deal, and I dropped the warranty when it expired. Maybe not my best ethical moment in retrospect, but I was in survival mode at the time, if that matters. Karma's since been well-repaid...
I'd bet the cost of the warranty is calculated in similar fashion to insurance rates, and of course the less they pay out, the better their bottom line. They may not be regulated as much as insurers, though. Haven't bought a warranty for anything since, however.
