I am thinking that looks like a SS head, raised ports, "stock" port volume, etc. The only thing I'm not sure is whether filling the pushrod hole on all four intake ports would be legal for SS. They allow a "repair" on two ports only. Did that end up being a W2 exhaust port design? It looks pretty close. What welding rod did you use? I am looking at something similar on the BB head.
