This intake was on my 383 in the Cuda and I can find that number but they all appear to be cast, this one is aluminum!
Has anybody seen one of these before?
It also has 383 HP and a pentastar.

It must be, I scoured the internet and all the ones I found were steel with 4 holes.

Looks just like an old Edelbrock I had on my 383 Dart. I ground the Edelbrock script off of it. Though the runners look more like an old Weiand...

Definitely an Offy intake. I wonder if somebody ground it down, crudely made a pentastar in it and stamped it to appear like a stock intake. I am supposing somebody may have used the intake to cheat in a stock class back in the day.

Even the direct connection stuff had the numbers cast in rather than stamped IIRC.

Hmmm, interesting. A close up of the area around the numbers does show a lot of "scratches". So it's a 5956 Offy.

It's possible to mount the big Holley 4500 on a number of Offenhauser manifolds with a special adapter. However, 360 Degree Equa-Flow Single Quad 4500 High-Rise intake manifolds are designed from the beginning specifically to fit the Dominator. The single plane design, split plenum, and tuned intake runners combine to produce a broad power band that runs from 3,400 rpm up to 8,900 rpm.

Edelbrock had a similar intake that would mount a 4500 Holley without adapters. It was based on the 4150/4160 Holley CH4B only with a 4500 flange. Model designation was CH440.
