Update
I finally got them done. After checking with 8 different places (I didn't check with Mike Roth) and getting everything from:
Asking for pictures and then getting no reply.
Being quoted $2500 per header.
Being told they couldn't use the materials I already had because their "weld specialist" is particular about his materials.
2 places were booked till well into 2016.
I settled on Quinn's chassis shop after seeing him recommended on YB, he is located west of Louisville KY, after initially contacting him he was able to get me in within 2 weeks, he had the car for 3 weeks, I'm happy with how they turned out and the cost was more than reasonable. Just waiting on the trim rings I ordered to finish it off.
Not that it matters but I thought it was cool that he is a Mopar guy, he's currently building a 25.5 chassis on his 68 dart to run in OSCA nostalgia true street with a turbo 392 gen III Hemi, he initially built a wicked LS based engine but couldn't bring himself to go through with it so he sold it off and built the 392.
