If I had a crankshaft in front of me I probably wouldn't have to ask some of these questions. Sorry to show my ignorance, but....

Are the "throws" on the crank all at 90 degrees to each other?

On one crankshaft revolution, do pistons 1-8-4-3 fire, and the next revolution 6-5-7-2 fire?

Each of the 4 throws are at 90 degrees to each other, for a total of 4 different (90 4 times = 360 degreees)
Since the 2 banks of cylinders are 90 degrees apart, one cylinder fires every 90 degrees. Each cylinder fires every other revolution, so it takes 2 complete revolutions to fire all 8 cylinders. 8X90 = 720 degrees.

Well you probably don't want to hear this little complication, but there are what used to be called "flat cranks" which have the throws at 180* I believe Ferrari does or did use these. At one time, a certain amount of drag racers used to experiment with 'em.

That would be basically the same as an inline engine crank. I always figured the reason the v8 cranks were offset like that was for engine smoothness or something to do with the way the crank gets worked under load...

Depends on the "inline" 6 cylinder engines are not 180, they are every third of a turn, IE every 120

4 whanger would be 180
