Does it go over the existing skin or does the original skin need to be removed?

Looks like a "filler" panel, covers the recessed part of the gate.

And since they are calling it a "skin", like the replacement "skin" for a door, it is a weld on part.

Just my take on it ...

You could just weld up your existing one...

I made my own smoothed tail gate on my 84. I welded a panel over the recess and reversed the tail gate handle.

i made my own smooth gate and put in a handle flip kit. sense i ain't going to be able to finish the truck, i have it up for sale along with getting rid of all my truck parts and the truck.
