Are yiou saying the hat was loose from the rotor?
Loose fit to the axle?
A gap between the rotor and brake pads?
Lugnuts tight?

My Wilwoods are prehistoric. The hats have never come loose.
Safety wired and Locktited. Though I doubt at the temps rotors run at if the locktite does much.

I had to make the axle bracket for the dbl caliper.i use the mopar green bearing axle ends but if you use a ford end i think thay have a dbl bracket but i dont have a part #.i think it was Dramperformance that said he would make a dbl bracket if someone wanted.
PS: i use 15" convo pro rim and clear brakes.
