This, they sell a pop rivet like tool to install new ones and the tool is real cheap. Careful when removing the old one so not to make the hole bigger.
And you might be able to tighten up the ones you have now with a tool.

Menard's also has the tool - at least the one nearest my house does. It's a Menard's branded tool, so I would guess they all carry it. It was in the same section as the pop rivet guns, and comes with 4 different sizes of the RivNuts, including the size needed for the side mirrors. I bought one 2 or 3 years ago from them and they only wanted $9.99 for it.

Wow that same tool is $34.99 in Canada at Princess Auto.
Figures too, cause I need one. The pass side mirror on my 74 has no inserts. looks like soemone just drilled the holes and used some clips on the inside to hold the screws on.

My car had the same issues as yours.
I bought the tool as suggested and used it to just tighten up the inserts that were all ready in the door. Worked perfectly, if that fails just remove the old ones and use the new inserts that come with the tool.

I did like RS23U1G suggested. Using the mirror's base as template, installed from inside the door. Displaces the load and is much more secure than using the rivet inserts. If you use the rivet inserts you can get the rivets from Resto Rick http://www.restorick.com/proddetail.asp?prod=MirrorThreadedInsertThey are different than the normal rivet since the heads are flater. The normal rivets head are higher/thicker. This may result in the base of the mirror not sitting flush on top of the door's mounting surface but rather on the tops of the rivets heads.

hm.what if you need to remove the mirror,then the threaded piece of flat steel will fall to the bottom of the door?good for temporary solution but not in the long run,my apinion,upiniun upinium...just a thought.
