Check the tires...or borrow a set of rims and tires to check it. I've got an 04 AWD. Stock 17" rims w/ 265 tires, no lift or torsion tweaks. For the last three years, I had a vibration from 45 on up. First, I had the left ft. half shaft replaced, it didn't fix it. Then, I thought it was the front driveshaft...it clicked when braking and accelerating from a stop. It was bad, but not the source of the vibration. Turned out to be bad belts in two tires. The tires had been balanced numerous times and were always fine...but one leaked down slowly and the other developed a bulge. Two new tires and the vibration was gone.

Correct-- the 04 and 05 Ram 1500s had an optional full-time or all-wheel drive 4x4 system. The transfer case drives the front wheels at all times with something like a 47% to 53% power split front to rear. When you want 4wd you turn the knob and get 50:50, and also have a low range.

I had the same problem with my truck and i had to take out the leveling kit to fix it.

I think you have to replace the front drive shaft. There is only a single u-joint at the diff side of the shaft, the xfer case end has a sort of CV slip-joint deal. That is where mine wore out and I had to get a new/ reman from Carquest.
Balancing wont matter when Xfer case end is worn.
