You need to get a hole in the nail to relieve the pressure. I have done it using a brand new Xacto knife. Hand on a flat surface, nail up. Place the Xacto point on the center of the nail straight up and down. Slowly rotate the Xacto, do not press it down, let the weight of it do the work. Quickly a micro hole will appear and the blood will escape.

it looks like the blood has already dried under the nail, how long ago did you do it? If it still fresh, I have used a small paper clip, bend one end straight and heat it up, then drill it through the nail to release the presure Either way you are going to be living with it for six months or more.
Better a finger nail, than the black eye I had from catching a medicine ball to the face........everyone felt they needed to ask how I got it.

Dad would take a small drill bit, and work it on a nail when he did that to make a hole. When I worked at the IH dealer, we were stocking tractor trailer springs on a rack, and I dropped one on one of my big toes (no steel toes shoes there). Hurt like nothing else, but I worked the rest of the day. When I got home, the wife saw me limping,and when I took my shoe and sock off, the front half of my foot was purple. So she said come on,we're going to the ER. Doc wanted to give me a shot in my foot for the pain, but I said I did this this morning, and had been on it all day and didn't need the shot. Came back from x ray, and I had shattered the bone in the toe. Doc said he was going to make a few holes in the nail to relieve the pressure ( I remembered dad's drill bit trick and expected the same kinda thing) Wife asked the Doc if it would hurt...Doc said "small pinch....Next thing I knew I thought he dropped another spring on me. I looked down and he had a soldiering iron. He did it again and another scream outta me. Doc said I need to to do this again....The wife ripped into him. The Hell you are, I thought you said it wouldn't hurt!

Even a clean 1/16 drill bit in your fingers will bore a hole and it will feel 100x better with the pressure off. I'm really reluctant to recommend anyone do "bathroom surgery"... the finger, nail AND drill bit all have to be surgically clean, not just wiped off on your pants! The best tool I've ever used to trephine (the technical term) a fingernail or toenail is essentially a fancy disposable soldering iron. It has a plastic sleeve containing a couple of batteries, and a tiny filament tip. Hit the button, tip heats orange, burn through the nail, blood comes out (sizzles on tip), patient feels better immediately. It only hurts them if you are careless and go too deep. The fingernail has no nerves - but the nail bed and fingertip sure do, and are already angry. (It's also silly to make them single-use. Nothing infectious could survive the 1700F temp).

Ladder slipped off the edge of the roof , broke 4 of the 5 metatarsal bones ... pins in all 4 hanging out the bottom of my foot so the pieces will mend somewhat straight ...
