I think that we have a winner. Not production car. They made 55 of them. I'd imagine there are one of one prototype show cars with one of one wheel covers and or rims. Here's the rim or predessor to it that 6bblgt mentioned got canceled before production in 1971 Others

Pretty sure NASCAR and NHRA had a minimum 500 production units, hence the '69 Charger "500" name as they had to build "500" of them. And there was an intent to only build "500" of each A12 6BBL Road Runners and A12 Six Pack Super Bees to meet NHRA'S production requirements (and then mess with NHRA on submitting specs as they felt a COPO Camaro with a 427 ZL1 that was allowed into to did have the documentation of "500" built or available at dealerships) quite funny and fun for the engineers at the time.
How many Hemi Darts were built, only 50? If so did it run in a production based class? That would make a difference.

The 1971 E-bodies had a cast aluminum 15" road wheel canceled very early on. Did any of the earliest cars have them installed? Chargers too, I forgot about them, but I located my dealership order book with the image;

EDIT: Apologize on the NHRA production count as it seems it was a minimum of (50) customer's names that had to be submitted to NHRA in order for the car to be legal in the Super Stock division. Hmmmm...I saw no nomenclature that required "production" as the criteria for the " what's the rarest Mopar wheel"...IMHO "55" units would denote "production" if that's YOUR requirement?,... wasn't "50 units" the NHRA's minimum requirement to allow the 68 SS Hurst cars to be considered "production units"?... IIRC "50 units" is considered the absolute minimum in the automotive production world to be rendered a "production unit"(ie: BTM Cheetah among many others)?...

EDIT AGAIN: I SHOULD HAVE READ THE ARTICLE I HAVE FURTHER TO GET TO THE 500 NHRA MINIMUM TO RACE A STOCK OR SUPER STOCK CLASS.
BOTTOM OF PAGE 2 (magazine Page 9) where it reads about the 500 units:

Or something unique made by Chrysler that just happens to look similar to later aftermarket rims?

I was thinking wheel, but including the cover is a good idea.
The 71 Sport Fury GT wheel is unique, and the wheel cover, actually it's a trim ring, only fits on that specific wheel; none other. With only 375 '71 GT's made and not all of them had these wheels and rings, I'm leaning on this one being the rarest. They were not limited to only the Sport Fury GT, but were an available option. Are you sure they were an "option", or available as an ordered part?
My info tells me they were "standard " on a 71 GT, or the Chrysler Rallye Wheel was optional.

I was thinking the same thing. I really like the 1970-71 Sport Furys and I think that one year only wheel gives the '71 a slight advantage in my book.
Dave Perhaps a slight advantage, but I really think the Turbine car wheel treatment is made even rarer, due to the fact 95% of the cars were later destroyed, with only a literal handful left to exist

Are you absolutely sure?... besides the fact they're not "production" pieces
