My 440 water pump had only one washer and that was on the bolt behind the nipple. This bolt was also approx. 1/4" longer than the other bolts. I can't say for sure it was originally like this from the factory.

Here we go!!! The Moparr Motors Chrysler Vehicle Restoration Reference Manual (Guide to B Body fasteners 1968-1970)has a note which says "The bolts in position #3 were consistently found with a washer" Position #3 is the one behind the nipple.
