A bolt goes through the top hole of the starter and a Lakewood adaptor gizmo goes through the bottom hole.

The adaptor for the lower hole has a bolt that goes through the lower ear on the block and a bolt-sized pin that goes through the lower bolt hole on the starter motor It's offset by about an inch from the lower bolt.

Make sure you use the part that holds the nose of the starter stationary too. I wires the starter gear out, so I could tighten the starter up with proper tooth engaugement. Of course the motor was out of the car and it was hard to see, but with the transmission out of the way, it was do-able.
