Based on what I've seen Terry is right on the money on all accounts. Concerning the Teleflex markings on the long cables, I've seen a dozen or so originals, every on had the lettering.

*Terry, those "Die Tabs" you mentioned are referred as "Ejector Pin Marks" within the molding industry, they are small round pins located inside the plastic injection mold which are spring loaded and pass through the mold, when the mold opens the springs release and advance the pins away from the mold cavity ejecting the part from the mold automatically so that a person does not have to pull them out between machine cycles.
