I think what he means is why does the positve not just short straight to the negative, even without the element in.
Yes, there is an insulator that supports the center contact, and keeps it from contacting the outer housing. I think it's porcelyn. I think they break and fall out after a while.
Contact is made through the lighter element itself, which (as mentioned) has a resistance to it.
