I had this happen exactly. The poreclain thread/fitting had a slight crack in it and caused my alt to over charge - saw the numbers on my voltmeter jump up to nearly 17!!! Pulled it apart, found a hairline crack in the threads. Replaced it, never happened again. Hope that helps.
