Does grinding them wear through the hardness? Do they have to be heat treated after they are ground?

Yes, that's what they do.
HOWEVER: there is no fixture that actually restores the original radius to the pad, the accuracy of the finished product depends largely on the skill of the operator.

Agreed, you need a very light touch/minimal material removal.

Rocker Arm Specialitys AKA RAS( not the new name, sorry) does complete blueprinting and rebush the rocker arms that need it or want them
Not cheap or fast though
