5.9 magnum cam sensor

How does it work? Is it a simple on/off once every time the distributor makes a 360 rotation? Would it work the same if rotated counter clockwise?

It's a hall effect sensor that sends a square wave signal to the ecu. It would not know or care which direction the distributor vane is being rotated.
