My 94 Ram with a 360 automatic with 212,000 miles will no longer go forward when in drive. It will go forward if I place the shifter in 1st but not while in drive. Reverse works okay. Could this be a band out of adjustment or is the high gear clutch pack gone?

well first off they do not last forever.

The high gear clutch pack does not engage till the truck is moving and it shifts into third . When you put it in manual low the rear band applies and holds the rear drum , the only thing I can think of is your overrunning clutch is not doing it's job of holding the rear drum while you are trying to start off in drive? But that doesn't make sense .

Sounds like overrunning clutch went out, either way pull out and tear down

that's my thought but the over running clutches don't look like they would fail in the OD trans by design ? I broke the output shaft on a 47RE powerbraking against the convertor with the trans in D and nothing happening to the over running clutch.

There are 4 elements that hydraulicly control forward gear positions. The direct(rear most) clutch is applied in all forward posistions,Therefore if the truck moves in forward,it works. The 3rd/reverse clutch is applied in 3rd and reverse. Reverse works so the clutch is attempting to function. However the pressure going to that clutch is higher in reverse. The low revese band is applied in 1st and reverse,therefore it works. The intermediate band is used for 2nd gear only,2nd is inop. However I doubt if the band is the problem as you have no 3rd either. If I had to bet the front planetary has stripped splines on the intermediate shaft. Either way it will need a complete tear down.
