note the 180 degree crack around the speedo cable input and the crack in the body just forward of that

Mine also let go in the lights at 120 mph about 2 years ago. It smashed the trans in two, beat the hell out the floor broke the shaft in two.
I upgraded to the 1350 u joints after and replaced the 8-3/4 for a Dana.
It looked like one of the straps came loose and spit the bearing cup out.

No way was it the bolts or the straps that failed...
I would say that a clip was not installed... OR ...
the grease fitting was cracking and finally let go
plus it might have had a vibration in the shaft to
help with the failure (grease fitting area).... I've
been using the same yoke for years and havent had any
problems...... I know some of the guys dont get the
warm and fuzzy feeling using the strap type but if you
look at the biggest diffs out there they use the straps.
you need to look at the joint and see that the cup
slides in OVER 1/2 the diameter so the load ISNT
applied to the strap..,. the strap is to hold the
DS from sliding forward thats all
EDIT
Also make sure your lock washers still have flex/spring
to them so they will lock, also dont think you need
to over tighten the bolts.... I use the standard 3/8
length wrench to tighten them.... they are hard
bolts but you dont need to get crazy on them
