Not much stuff good with a 100K miles on a 4.7.
Start by pulling the valve covers and make sure all the lifters are still in place and functioning correctly.
Then is probably time to replace the plastic timing chain guides. The worn guides allow the timing chains to flop around and can change the valve timing by a few degrees.
Hopefully it has not been overheated and blown a head gasket, or wiped out the bearings on the crank.

Our 2005 Dodge Ram 1500 with the 5.7 Hemi had a bad seal on the EGR tube. It was leaking on the intake end on the EGR tube.. New seal and all working again as it should.

Did you pull the valve covers?
