Having an issue with the rear axle adjuster/locking pawl. What can cause the locking pawl to get bent out of shape. This has happened twice. End play was set within the FSM range.

I would assume that it had to much end play, not enough preload, allowing the wheel bearing to try and moved the outer race (in a left hand turn) in the rear end housing which was trying to rotate the axle retainer plate which twisted the lock that was trying to hold the retainer work scope
You need to remember that the tapered Timken wheel bearings on your axle are used in the center section with a lot of preload on them also so don't be afraid to tighten the end play up some more on the end paly adjuster to solve your issue wrench twocents
I set mine up at .001 to ,003 end play, no more than that tsk scope
