Did non-AC cars come with a water flow valve in the heater hose?
If not, why?

I want to say no, they didn't.
The blend door directed either fresh air or heated air into the cabin.
since you didn't have to fight the heat with a/c no reason to add a valve.

Where'd the factory typically pull vacuum for the vacuum actuated controls in the heater/ac system? Is it just pulled from the intake manifold?

Yes, screw in fitting in back of intake runner where brake booster hose usually goes. a small tube for the heating system. Can be just the small tube straight up if car has no power brakes, otherwise the large one straight up and the small one comes out of the side where the hex is for the wrench.
