Where should I connect hot from the electric choke? I understand it should be from a source that is hot only when the engine is running, but am not sure where that would be? The ballast resistor?? If so which side??

factory took it from the dark blue wire off the same side as the ballast resistor.

from the choke run a wire down several inches & tap into the wiring harness running along the inside edge of the valve cover & tap into the blue wire thats going to the alt field lead (on a 2 field terminal alt)

What year? If its 73 or newer its already there. If its older than 73 the previous posters gave you the correct way to do it.
