Is there some sort of a transition collar or seal that goes from the end of the defrost duct work to the heater box? If so could someone a picture?

Nope.

The open area acts as a 'mixer' drawing in cabin air to warm it too.

I recall there being a rectangular shaped piece of open cell foam 'sandwiched' in that area. Could have been a later year add-on. I doubt chrysler would have left that space empty. Open cell foam will still allow air to be drawn into it, if that's your goal.
